What Is Gynecomastia?

Gynecomastia is the enlargement of the male chest caused by excess glandular tissue, fat, or both. It often presents as chest fullness, puffy nipples, or a rounded appearance that does not respond to diet or exercise.

In most cases, the firm gland beneath the nipple cannot be reduced without surgery. Removing this tissue is what allows the chest to become flat and more defined.

Who Is a Candidate for Gynecomastia Surgery?

You may be a candidate if you have:

  • chest fullness that does not improve with diet or exercise

  • puffy or protruding nipples

  • firm tissue beneath the nipple

  • a soft or rounded chest appearance

  • uneven or asymmetric chest contour

  • loose skin after weight loss

The best candidates are in good overall health and at a stable weight.

 

Massive Weight Loss and Advanced Gynecomastia

Men who have experienced significant weight loss, whether through GLP-1 medications (such as Ozempic, Wegovy, tirzepatide, or Zepbound) or bariatric surgery, often develop excess chest tissue and loose skin that does not resolve on its own.

In these cases, the chest can appear deflated, sagging, or more breast-like rather than flat and defined.

Types of Gynecomastia Correction

Liposuction Only

Used when the chest fullness is primarily due to fat. This is appropriate for patients with good skin elasticity and minimal glandular tissue.


Gland Excision

Required when firm glandular tissue is present beneath the nipple. This is the tissue responsible for puffy nipples and cannot be removed with liposuction.


Liposuction + Gland Removal

This is the most common approach. It allows complete removal of the gland while contouring the surrounding chest for a smooth, masculine appearance.


Skin Tightening or Skin Removal

In cases with loose or excess skin, particularly after weight loss, additional skin tightening or excision may be necessary to achieve a flat contour.
