Cost of Surgery For “Self-Pay” Patients
Dr. Garramone’s prices for his surgical fee are reflective of his vast experience and expertise in the area of chest surgery. Simply choosing another surgeon because they have lower costs may end up costing you significantly more in the long term. Most surgeons who charge lower prices are not as experienced, have poorer results, and can mislead patients about their final results. You may need multiple revisions to fix another surgeon’s poor technique, and this can add up financially very quickly. Dr. Garramone is considered the best in the world at FTM Top Surgery, with low revision rates and low complication rates. Our office staff are professional and experts in patient care. Our hospital facilities are considered the best in South Florida, and the clinical staff and Anesthesia department are experts in clinical and anesthesia care. The cost is justified and definitely worth it! Don’t pick another surgeon because they are cheaper. Cheaper usually isn’t very good, but very good is never cheap. All FTM Top Surgery Procedure costs are estimates for information purposes only and are subject to change without prior written notification. The Hospital and Surgery Center may changer their rates at any moment, as they are independant and separate facilities from Dr. Garramone’s office.
All surgeries are performed as an outpatient procedure and do not require an overnight stay in a hospital unless otherwise notified. If you are traveling from out of town, Dr. Garramone requests you stay in our local area for at least one week so he may monitor you during the early postoperative period.
All costs include preoperative consultation fee, postoperative physician care, and a letter written by Dr. Garramone stating you have had irreversible sexual reassignment surgery. Revision surgery is only covered if related to the prior surgery performed by Dr. Garramone with a reasonable amount of time as determined by Dr. Garramone on a case by case basis. If revisions must be performed in a hospital or surgery center, only Dr. Garramone’s fee may be waived. The patient is responsible for all outside operating room, pathology, and anesthesia fees.
FTM & FTN Top Surgery Double Incision Method
Cost will be determined at time of your consultation: is Dr. Garramone’s Surgical Fee for “self-pay” patients varies depending on the size and difficulty of the procedure, which will be determined during your consultation. The Hospital & Anesthesia Fee is separate. The hospital requires all breast tissue to be sent for pathology examination, our office has begun collecting $300 for routine pathology examination of the breast tissue. Any special studies of pathology will be billed to the patient directly. The estimated total fees vary and include the hospital facility fee, anesthesia fee, and Dr. Garramone’s Surgical Fee. Prices can vary depending on hospital, when the procedure is scheduled, or if the hospital increases their fees. This applies to all Breast sizes and includes liposuction of the lateral breast if necessary. Dr. Garramone’s fee must be paid to our office at least 4 weeks prior to the surgery date.The Hospital & Anesthesia Fees must be paid directly to the hospital by the patient on the date of surgery. The Hospital & Anesthesia Fees are based on an hourly rate and the Hospital will charge the patient additional fees above the allotted time. This is rare, but we must be clear with you about the Hospital Fees. The Hospital or Surgery Center Fees can vary by date and availability.
FTM & FTN Top Surgery "Keyhole" Method
Cost will be determined at time of consultation: is Dr. Garramone’s Surgical Fee for “self-pay” patients varies depending on the size and difficulty of the procedure, which will be determined during your consultation. The Hospital & Anesthesia Fee is separate. The hospital requires all breast tissue to be sent for pathology examination, our office has begun collecting $300 for routine pathology examination of the breast tissue. Any special studies of pathology will be billed to the patient directly. The estimated total fees vary and include the hospital facility fee, anesthesia fee, and Dr. Garramone’s Surgical Fee. Prices can vary depending on hospital, when the procedure is scheduled, or if the hospital increases their fees. ManSculpture® Procedure Body Masculinization (liposuction of abdomen, flanks (love handles), and inside or outside thighs also known as MaleSculpture®
Cost will be determined at time of consultation: This can vary depending on the size and difficulty of the procedure, which will be determined during your consultation.This is in addition to Top Surgery when performed at the same time. Many patients choose to have this be performed at the same time to better contour their body to a more masculine form. ManSculpting is also more cost effective to have performed at the same time as Top Surgery. The Hospital & Anesthesia Fees must be paid directly to the hospital by the patient. You will be given the total amount due to our office 4 weeks prior to the date of surgery, and the amount due to the Hospital on the day of surgery. Pricing is per area. Typical areas are the abdomen, flanks, and outside thighs. Dr. Garramone’s surgcial fee vary. The additional time for hospital fees will be added during your consultation for accuracy.
FTM & FTN Top Surgery Revision from another doctor "Botched Top Surgery"
Cost will be determined at time of consultation: is Dr. Garramone’s Surgical Fee for “self-pay” patients varies depending on the size and difficulty of the procedure, which will be determined during your consultation. The Hospital & Anesthesia Fee is separate. The hospital requires all breast tissue to be sent for pathology examination, our office has begun collecting $300 for routine pathology examination of the breast tissue. Any special studies of pathology will be billed to the patient directly. The estimated total fees vary and include the hospital facility fee, anesthesia fee, and Dr. Garramone’s Surgical Fee. Reserving a Surgical Date
To reserve a surgical date for FTM & FTN Top Surgery a $1,000.00 (U.S. Funds) Non-refundable surgery reservation fee is required. To reserve a surgical date for the ManSculpture® Procedure an additional $1,000.00 (U.S. Funds) Non-refundable surgery reservation fee is required. Upon making a reservation we will email you a preop packet with our financial policy, which must be signed and returned to our office immediately to confirm your scheduled reservation. Final payment is due four weeks prior to surgery. We accept all major credit cards, Zelle, certain medical loan payments, or cash. We do not accept Personal Checks or medical insurance. All preop labs, prescription medications, mammograms, pathology fees, and postoperative compression garments are not included in the above fees. The hospital requires all breast tissue to be sent for pathology examination, our office has begun collecting $300 for routine pathology examination of the breast tissue. Any special studies of pathology will be billed to the patient directly. We will provide prescriptions (to be filled by the patient) for all medications, labs, and mammogram studies. All specimens of breast tissue removed during surgery must be sent to pathology for microscopic evaluation to rule out any evidence of breast tissue disease.
While we do try to keep the costs for surgery low, we do not have any control over changes in the hospital, anesthesia, or pathology fees; nor do we have any control of increased costs to our practice and overhead from city, state, and county regulations, along with medical services and medical suppliers. These entities provide a service for our practice and are not owned or operated by our practice. All fees are subject to change without notice. In the event of any fee changes, we will update this page and contact patients as appropriate. If you receive a bill from the Hospital, please contact the Hospital first, and please understand it is your responsibility to settle any bills received from outside our office which may include Hospital or Pathology Fees.

At The Garramone Center, we want to ensure our patients are well-informed when navigating the insurance process for surgical procedures. Please note that our office is not contracted with any insurance providers and we are considered an out-of-network provider. If your insurance plan allows for out-of-network benefits, we can assist you in obtaining a Letter of Guarantee (LOG), which ensures your insurance company agrees to cover the surgeon’s fees for your procedure.
Here’s an overview of the steps involved for both you and our office to secure a Letter of Guarantee, ensuring compliance with federal and Florida laws.
Steps for Patients
1. Contact Your Insurance Company:
• Confirm if your plan offers out-of-network benefits.
• Ask about any deductibles, copayments, or coinsurance.
• Verify if a Single Case Agreement (SCA) is possible for your procedure due to its medical necessity.
2. Provide Us with Insurance Details:
• Share your policy number, group number, and insurance contact information.
• Let us know if you have already obtained any authorization numbers.
3. Understand Your Financial Responsibility:
• Be prepared to make a prepayment for the procedure, which will be applied to your costs if insurance does not cover the full amount.
• Review our Good Faith Estimate for a transparent breakdown of all costs, including potential out-of-pocket expenses.
Steps Our Office Performs
1. Prepare and Submit Documentation:
• We will compile all necessary paperwork, including a Letter of Medical Necessity, procedure details (CPT and ICD-10 codes), and a cost breakdown.
• Our team will submit a formal Request for a Letter of Guarantee to your insurance company.
2. Coordinate Preauthorization:
• We’ll work with your insurer to secure preauthorization and negotiate a Single Case Agreement (SCA) when applicable.
3. Ensure Transparency:
• We will explain your financial responsibilities clearly, including deductibles, copayments, and the application of any prepayments.
4. Follow Up:
• Our staff will track the status of your LOG and provide updates on approvals, ensuring all steps are compliant with federal and Florida laws.
